SERUM NT-PROBNP IN CHILDREN WITH VARIOUS LEFT TO RIGHT SHUNT CONGENITAL HEART DISEASE

Background : N Terminal pro B-Type Natriuretic (NT-proBNP) levels are elevated in children with left to right shunt (LtR) congenital heart disease (CHD). However, the benefit of this biomarker as a diagnostic tool of heart failure or pulmonary hypertension in congenital heart disease is still need further investigation. As far as we know, reports in the literatures about benefit of NT-proBNP as a cardiac biomarker in pediatric cardiology are scarce. The aim of this study is to compare the NT-proBNP levels among various types of LtR CHD Material : We conducted a cross sectional study involving 39 children (23 males 16 females) who admitted to pediatric cardiology clinic of the Dr. Sardjito Hospital in May 2018. There were 5 types of defect, i.e. atrial septal defect(ASD), ventricular septal defect (VSD), patent ductus arteriosus (PDA), combined ASD and PDA, and combined ASD and VSD based on echocardiography. The NT-proBNP levels were measured using electrochemiluminescence immunoassay at the recruitment day Results : There were 20 (51%) children presenting with VSD, whereas PDA, ASD, combined ASD and PDA, and combined ASD and VSD were 10(27%), 6(15%), 2(5%), and 1(2%) respectively. The median (interquartile range) of NT-proBNP in all subjects was 342.9 (153.1-731.2)pg/dl. Among children with LtR shunt the highest levels of NT-proBNP was found among PDA 550.6 (329.3-2195) pg/dl and ASD 321.5 (169.8-1069.7)pg/dl followed by VSD 257.6 (165-356)pg/dl. Compared to normal levels in children of particular age, the NTproBNP levels among LtR shunt CHD were higher Conclusions : NT-proBNP levels in children presenting with LtR shunt congenital heart defects are higher than normal children of particular age. Further investigation is needed to evaluate the benefit of NT-proBNP as a biomarker for LtR shunt CHD in childrenv
