CHARACTERISTICS OF PERSISTENT PULMONARY HYPERTENSION OF THE NEWBORN IN SITI KHODIJAH HOSPITAL SEPANJANG SIDOARJO

Background : The incidence of persistent pulmonary hypertension of the newborn (PPHN) in preterm, term or near-term infants is reported 6.8 per 1000 live births. Clinical manifestation PPHN widely different variety and cause morbidity and mortality. The aim of this study is to describe the characteristic of PPHN.
Material : A descriptive study in infants with PPHN at Neonatology Ward Siti Khodijah Hospital Sepanjang conducted between July until December 2017. Variables of the study were: characteristics of the subjects: gender, gestational age, body weight, length of stay, management of PPHN (O2 continuous positive airway pressure (CPAP), sildenafil, and dobutamine) and outcomes (recovery, recovery with pulmonary hypertension, died).
Results : Twenty-five infants were enrolled. The characteristics were male 16/25, female 9/25, the modus gestational-age 37 weeks, mean body weight 2668 (SD 599.8) gram, and mean length of stay 7.8 (SD 2.39) days. The co-morbidity causes were meconeal aspiration syndrome (7/25), respiratory distress syndrome (RDS) (23/25), lungs disorder (8/25), and hypoglycemia (10/25). The management of PPHN were sildenafil (4/25), sildenafil + dobutamine (10/25), sildenafil + CPAP (1/25), and sildenafil + CPAP + dobutamine (10/25). The outcomes were 4/25 complete recovery and 21/25 with mild-moderate pulmonary hypertension.
Conclusions : The characteristic of the infant with PPHN were male, term-infant, normal birth weight. RDS is a co-morbidity of PPHN. The common management was the combination therapy of sildenafil + dobutamine and sildenafil + CPAP + dobutamine. The majority outcomes were mild pulmonary hypertension.
