ATTENTION-DEFICIT HYPERACTIVITY DISORDER AND CHILD MALTREATMENT A CHINESE POPULATION STUDY

Background : Attention-deficit/hyperactivity disorder (ADHD) and child maltreatment are both major health and psychosocial problems in childhood. Clinical observations support an association between ADHD and child maltreatment. However, despite an alarming parallel rise in the incidence of child abuse and ADHD diagnoses in Hong Kong in recent years, the relationship between ADHD and child maltreatment, and its possible mechanisms in Chinese societies, remains an important knowledge gap.
Material : This is a population representative survey conducted in Hong Kong, China. Schools were sampled from all five regions of Hong Kong. All Grades 1–3 (6–10 years) students in the participating schools were invited to participate. Parents of these children completed validated questionnaire on ADHD and child maltreatment as well as other potential confounders, such as family income and family history of ADHD/psychiatric disorders.
Results : 7585 students (54.3% boys, mean age [SD] 8.15 [0.88] years) were recruited from 51 primary schools. The estimated prevalence of ADHD, based on the parent-report questionnaire, were 7.0% (girls 4.8%; boys 9.0%). ADHD risk Presence of minor (risk ratio 1.77, 95% CI 1.38–2.26, p < 0.001) and severe physical abuse (2.47, 2.02–3.04, p < 0.001) were associated with ADHD risk, as well as psychological abuse (1.58, 1.11–2.27, p = 0.01) and neglect (1.48, 1.19–1.83, p < 0.001) after adjusting for age, sex, family income, parental history of mental disorders, and sampling region. Similar findings were observed when we used parent-report ADHD diagnosis as outcome.
Conclusions : This is a positive association between ADHD and child maltreatment in Chinese society. This indicated the need for active maltreatment screening amongst ADHD patients and the formulation of integrated treatment programmes.
