Effects of sleep hygiene treatment for long sleeper and blue light therapy on school non-attendance due to wake-up difficulty in students

Background : In recent years, there are increasing numbers of school non-attendance due to wake-up difficulty in Japanese students. In our department, we are conducting treatment with these students not only with sleep hygiene treatment for long sleeper but also with home blue light (BL) therapy. In this study, we aimed to evaluate the effect of sleep hygiene treatment and home BL therapy with medication on school non-attendance in Japanese students. Material : 26 students with school non-attendance aged 13 to 18 years old who were suspected hypersomnia and conducted the polysomnography (PSG) and the multiple sleep latency test (MSLT) in our department were enrolled. At the first visit of our department, we calculated average sleep time per a day from sleep diary and conducted sleep hygiene treatment. All patients prescribed aripiprazole (1-3mg/day) and started home BL therapy from the first visit of our sleep clinic. We compared questionnaires; Pittsburgh Sleep Quality Index(PSQI), Epworth Sleepiness Scale(ESS), Self-rating Depression Scale(SDS) and MOS 36-Item Short-Form Health Survey version2(SF-36ver2), before and after treatment Results : 5 of 26 students were diagnosed with narcolepsy without cataplexy(ICSD-2).11 of 26 students were diagnosed with idiopathic hypersomnia(ICSD-2). 17 of 26 students were diagnosed with long sleeper (ICSD-2). Comparison between before and after treatment, PSQI[t(25)=3.54, p<0.01], ESS[t(25)=2.66, p<0.05], SDS[t(25)=4.00, p<0.01] were significantly improved. In SF-36ver2, Physical function[t(25)=2.42, p<0.05], Role physical[t(25)=4.02, p<0.01],General health[t(25)=2.28, p<0.05],Role emotional[t(25)=2.96, p<0.01],Mental health[t(25)=2.93, p<0.01] were significantly improved. Of 26 students, wake-up difficulty was improved in 24 students and school non-attendance was also improved in 20 students. Conclusions : This is the first report to evaluate the effect of sleep hygiene treatment for long sleeper and home BL therapy on school non-attendance. Sleep hygiene treatment for long sleeper and home BL therapy were effective for school non-attendance in Japanese students.
