INFLAMMATION-BASED SCORES AS A PROGNOSTIC FACTOR FOR SURVIVAL IN PEDIATRIC OSTEOSARCOMA

Background : Osteosarcoma is the most common primary tumor of bone in adolescents and young adults. Systemic inflammation responses have been associated with cancer development and progression. Inflammatory-based scores such as: neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), lymphocyte-to-monocyte ratio (LMR), and neutrophil-to-platelet score (NPS) have been proposed to predict clinical outcome in many type of cancers. The aim of our study was to evaluate whether NLR, PLR, LMR, and NPS could be used as prognostic factors in pediatric osteosarcoma Material : Study participant were identified from a retrospective cohort of 41 children with osteosarcoma during the period 2010-2017. Data for the variables were obtained from medical records and analyzed in bivariate analyses. Overall survival (OS) was plotted using the Kaplan-Meier method and evaluated by the log-rank test. Statistical analyses were performed by SPSS and STATA statistical software Results : Our results showed that there were no association NLR, PLR, and LMR with time of event (dead). NPS was significantly associated with failure (P<0.05). High NPS was an independent predictor of poor outcome compared to both low NPS ((HR=3.94, 95% CI= 0.94-12.76, P=0.04) and medium NPS (HR=6.26, 95% CI= 1.30-26.40, P=0.012). Conclusions : Our study revealed that NPS might be used as independent prognostic factor for pediatric osteosarcoma. Clinical utility of this prognostic markers should be validated in larger pediatric studies
