Palliative Care at Dr Cipto Mangunkusumo National General Hospital, Jakarta, Indonesia

Background : Pediatric palliative care in Dr Cipto Mangunkusumo Hospital, just begun in 2015 and flourishing its coverage until now. The palliative care team consists of caregiver, family members, pediatricians, nurses, psychiatrist, chaplain, social workers, dietician, physical therapist, and pharmacist. This study described the characteristics of all children referred to children palliative care in Dr Cipto Mangunkusumo National General Hospital from 2015-2017.
Material : This is a retrospective study of children aged between 0-18 years with chronic diseases who got palliative care at Dr Cipto Mangunkusumo National General Hospital between 1st January 2015 and 31st December 2017. We collected the data from medical record and describe the characteristics of them.
Results : There were 140 children with range of age one month old to 18 years old and male 62% Most of them referred from Jakarta (60%), Sumatra Island (17,1%), and other provinces in Java Island (15,7%). The majority of diagnoses were cancers (65,7%), congenital malformations (7,9%), and chronic kidney disease (5%). Most common symptoms were pain due to cancer pain, dyspnea due to lung metastatic or cardiorespiratory problems, decubitus ulcer due to immobilization, parental anxiety, and depression. Their functional status was impaired and fully dependent to caregiver. By end of study, 74,3% were died, 9,8% referred to another hospitals, and 22% were discharged. The median of length of stays is 20 days with IQR 24,7 days.
Conclusions : Children referred for palliative care in Dr Cipto Mangunkusumo National General Hospital was increased in numbers. Mortality rate was high due to advanced and complicated disease. Multidisciplinary approach are needed to optimize palliative care outcome.
