The First Case of Neonatal Tetanus in Maumere (Rural Area) in 2017 – a Case Reportt

Background : Neonatal tetanus is a fatal disease caused by Clostridium Tetani. It is still prevalent in developing countries such as Indonesia. In 2016, there are 14 cases of nenonatal tetanus in Indonesia, none of them found in Maumere.
Case Presentation Summary : This case was an eight-day-male baby presented with progressive difficulty in feeding, seizure and asphyxia. The baby was afebrile. He was born at home without health workers. The umbilical cord was cut by the mother using a lumber. The mother has never been immunized against tetanus. No ulcer found in periumbilical cord. There was no gram staining nor culture could be done in the hospital. Hence, the diagnosis was made by history and clinical findings. The patient was hospitalized for 35 days and discharged in good condition.
Learning Points/Discussion : It is important to explore all the history and clinical foundings in order to allow its ealy diagnosis and appropriate treatment, especially in rural hospital thas has limited laboratory access. This case has shown the importance of tetanus toxoid vaccination during antenatal care and healthcare delivery.
