Hemorrhagic varicella in immunocompromised children a Case Report of 16-year-old girl

Background : Infections from Varicella-Zoster Virus (VZV) are usually considered as benign infections and have a low mortality rate in children. In contrast, varicella infections are potentially life-threatening in immunocompromised patients.
Case Presentation Summary : A 16-year-old girl was referred to the Emergency Room at a tertiary care hospital in Jakarta because of hemorrhagic vesicles around her body. She complained about the symptom 14 days before admission and the use of symptomatic drugs didn’t relieve it. She has had stage 4 osteosarcoma since 6 months ago and underwent chemotherapy. Thirteen days before the complaint, she had history of contact with her cousin with varicella. Five days after contact, she underwent chemotherapy and three days after chemotherapy, she developed high fever and clear vesicles appeared all around her body. The vesicles were itch, pain and became hemorrhagic in day six, after chemotherapy. Diagnosis of hemorrhagic varicella was made and isolation room, intravenous acyclovir, intravenous immunoglobulin, antibiotics, and symptomatic drugs were given. The intravenous acyclovir were given for 10 days according to the recommended use and she made a good recovery.
Learning Points/Discussion : Hemorrhagic varicella is one of the complications in immunocompromised children. Early diagnosis and prompt therapy may reduce mortality and morbidity of varicella in immunocompromised patients.
