Correlation Between Serum’s Calcium Levels and Dengue Severity Levels in Children Measured Using Pongpan’s Dengue Severity Score

Background : Indonesia is an endemic country for Dengue viral infection, which spreads evenly all across the country. The ability to differentiate mild and severe degree of Dengue viral infection will lower the disease’s mortality rate. One of the component in human’s blood that can differentiate Dengue viral infection’s degrees is serum's calcium level. A change in calcium homeostasis will affect cardiac function and circulation, and therefore, could take part in Dengue Shock Syndrome patophisiology.
Material : A cross sectional study, which was conducted in 146 subjects, with inclusion criterias such as follows ; children aged 2-14 years old, diagnosed with Dengue Viral Infection, and were admitted in the Pediatric Ward/ High Care Unit/ Intensive Care unit of Jombang Regional Hospital during 2017. Those samples were then categorized to Dengue Fever, Dengue Haemorrhagic Fever, and Dengue Shock Syndrome according to Pongpan’s Dengue Severity Scoring, which parameters are age, presence of hepatomegaly, systolic blood pressure, white cells count and platelets count. Pearson’s correlation test was used to analyze the correlation between serum’s calcium levels and Pongpan’s Dengue Severity Score.
Results : The sample size was 146 and the mean age was 8,2 years (range 2–13,9 years). The majority of the patients were males (n = 79, 54.1%). According to Pongpan’s Dengue Severity Scoring, the proportion of dengue severity levels in this study were as follows; 26,7% suffered from Dengue Fever (scored <2,5), 44,5% suffered from Dengue Haemorrhagic Fever (scored 2,5-11,5) , and 28,8% suffered from Dengue Shock Syndrome (Scored >11,5). The mean serum free calcium level of the samples was 8.2 mg/dL (range 5.87–10.2). There was significant correlation between calcium serum levels and Pongpan’s dengue severity score (r=-0.757; p =0.000)
Conclusions : This study concludes a significant inverse correlation between calcium serum levels and dengue severity levels in children which was measured using Pongpan’s dengue severity score. This can help healthcare provider to recognize dengue infection’s severity level more accurately by using both Pongpan’s scoring and serum’s calcium level, and therefore can lower the morbidity and mortality rate of dengue disease itself.
