ACUTE KIDNEY INJURY IN DENGUE INFECTION AT DR M DJAMIL HOSPITAL

Background : Acute kidney injury (AKI) is a serious complication of dengue infection. AKI in dengue infection result of various mechanism especially caused by capillary leakage and loss of fluid from intravascular leading to shock, which may lead to decreased kidney perfusion. The aim of this study was to determine the incidence and relationship between AKI with severity of dengue infection in children.
Material : A cross sectional study, in March to May 2018 with subjects were 1 month old to 18 years old with dengue infection admitted to Dr. M. Djamil Hospital. The subject was classified as non shock dengue infection and dengue shock syndrome (DSS). Subject with prior renal impairment were excluded. Glomerular filtration rate (GFR) was calculated using the Schwartz formula, AKI was identified and staged in accordance with pediatric RIFLE criteria, and statistical analysis using Kruskal-Wallis Test.
Results : Out of 24 patients, the mean age was 6.85 years with girl predominantly 17 (71%). There were 8 (33%) classified as non shock dengue infection and 16 (67%) as DSS. Higher level of creatinine and urea were found among DSS than non shock group. Out of DSS group, higher level of creatinine and urea were found during shock episode than 48 hours after shock resolved (creatinine 0.55 mg/dl vs 0.51 mg/dl, p = 0.359; urea 25.2 mg/dl vs 21.9 mg/dl, p = 0.191), but the difference was statistically not significant and none of our patient classified as AKI.
Conclusions : There was an accordance between higher level of creatinine and urea with the severity of dengue infection in children, but statistically not significant.
