AN INCREASED IN MEAN PLATELET VOLUME AS A PREDICTOR OF MORTALITY IN SEPTIC CHILDREN

Background : Sepsis is a life-threatening condition of organ dysfunction due to infection. Hematologic systems are the impact of potent inflammatory process of sepsis. Recently, many studies have been conducted to investigate mean platelet volume (MPV) as indicator of sepsis. MPV is a measurement of the average size of platelet that represent platelet function and activation that usually reported in complete blood count test. However, their prognostic value in pediatric sepsis remains unclear. This study aims to investigate the role of MPV as predictor of mortality in children with sepsis.
Material : Cross-sectional study was conducted on septic children in H. Adam Malik General Hospital and University of Sumatera Utara Hospital. Evaluation of MPV value was measured on the first day (0 hour) and 72 hours after sepsis was diagnosed.
Results : A total of 43 subjects were enrolled in this study. As many as 23 subjects (53.5%) suffered septic shock. MPV value at 0 hour and 72 hour in the septic shock group was significantly higher than sepsis group (0 hour, 9.4 ± 0.8 fL vs 10.6 ± 1.1 fL, p <0,001; 72 hour, 9.75 ± 0.76 fL vs 11.8± 0.9 fL, p <0,001). MPV value also significantly higher in non-survivor than survivor group, especially at 72 hours (9.7 ± 0.92 fL vs 11.8 ± 0.94 fL, p <0,001) with tendency of mortality 4.3 times higher in patient with MPV value ?10.8 fL at 72 hours while 2.62 times with MPV value ? 9.85 fL at 0 hour.
Conclusions : Septic shock patients had an increased in MPV values. A statistically significant difference of MPV value was found between survivors and non-survivors therefore MPV value can be used as prognostic marker to predict mortality for children with sepsis.
